NYC Medicaid Ride Checklist

Getting to a medical appointment can take planning. New York’s Medicaid Transportation program may arrange non-emergency rides to covered medical appointments at no cost to eligible members when a trip is approved. The program is for planned medical care; if you have an emergency, call 911.

1. Confirm that the trip is for medical care

Start with the appointment itself: the date, time, provider’s name, and destination address. New York State describes Medicaid transportation as a benefit for medical appointments such as primary care and dental visits. It is not a general transportation service for non-medical stops such as pharmacies, grocery stores, gyms, or schools.

2. Request the ride early

When possible, ask for transportation at least 72 hours before the appointment. Have your Medicaid identification number ready, along with the appointment time, pickup address, destination, and the name of the medical provider. New York State’s managed-care member handbook also advises members or providers to call at least three days in advance when possible.

3. Share what the scheduler needs to know

The transportation broker arranges the most medically appropriate and cost-effective level of service. Depending on the approved trip, that can include public transit, taxi or livery, ambulette, ambulance, or mileage reimbursement for a personal vehicle. Describe any transportation needs clearly so the scheduler can arrange the appropriate service. Do not assume that a particular vehicle type or service level is automatically covered.

4. Keep the ride details together

Write down the scheduled pickup time, destination, and any confirmation details you receive. Keep your phone available on the day of the appointment, and make sure the pickup address is clear and accessible. If your appointment is rescheduled or canceled, contact the transportation scheduler as soon as you can so the trip can be updated.

5. Ask about a provider preference without assuming an assignment

If you would prefer Agape Luxury Corp for an approved ride, tell the transportation scheduler and ask whether your preference can be noted. Provider availability, the approved service level, and the transportation broker’s arrangements determine the final assignment, so a request is not a guarantee. Asking early gives the scheduler the best information to work with.

6. Keep emergencies separate from scheduled transportation

Medicaid transportation is designed for non-emergency medical care. If you have a medical emergency or believe you need emergency transportation, call 911. For a routine appointment, use the current contact information on the New York State Medicaid Transportation page or the Medical Answering Services portal to begin the scheduling process.

For medical transportation information about Agape, visit the Medical Transportation page. For benefit rules, eligibility, or trip approval, rely on the transportation broker and New York State Medicaid’s current guidance.
